    yes, I think you are quite right.. by reading the specification, it seems
    like xsl:output instructions should be merged with imported documents..
    however, from mod-xslt, I'm just an user of the ``mediaType'' value
    of the stylesheet struct... so, I'm either wrong using that field
    of the struct, or it's a problem either in libxml or libxslt.

    Can you remember which versions of libxml and libxslt were you
    using with previous versions mod-xslt?

    Right now, I just documented the problem in the FAQ .. and taken
    a deeper tour in libxslt sources .. certainly, mediaType is not
    referenced often, and

    [...]
    YES Literal Result Element as Stylesheet
    YES Embedding Stylesheets
    NO mediaType
    [...]

    in FEATURES does not look too encouraging...
